Medical Records

As a patient, you always have the right to your medical records and our Health Information Specialist team is here to expedite the process. To obtain your medical records, you will need to sign and date a medical release form. This form may be mailed, faxed, emailed to you or you may come into the office to sign the release. Your records may then be mailed or faxed within 30 days from the request or you may receive them at the time you come into the office to sign the release.

There is no fee for your personal medical records. If the records are not requested by the patient, the fee is $1 a page for the first 30, then .25 a page after.
